We are currently recruiting for a Nursing Supervisor for our PACE facility in our Home Care department. Our Home Care Supervisor assumes leadership of a dedicated team of committed CNAs to meet the needs of fragile and elderly seniors.

Under the direction of the Center Director, oversees the day-to-day operations of the In-Home Services Program for the St. Paul‚s PACE. Directs and supervises RN’s, L.V.N‚s, Home Aides, and Scheduler of the In- Home Services. Provides supervision visits to home care staff in the field quarterly to evaluate skill and performance. Assists Center Director in policy formulation and program development and implementation. Participates in the establishment of annual budgets and financial objectives. Ensures compliance to company policies and procedures, and guidelines. Performs initial assessment annual reassessment of participants in their home care environment and coordinates plan of care with appropriate resources. Acts as the liaison between the contracted home health agency and St. Paul‚s PACE Ensures compliance with staff competency requirements. Maintains the participants‚ medical record with thorough documentation. Communicates participants‚ changes and progress to appropriate staff.

Skills/ Requirements
* Broad knowledge base of health care problems of the frail elderly.
* Knowledge of common safety hazards and precautions to establish a safe working environment.

* Skilled in identifying problems and recommending solutions.
* Knowledge of home care services principles and practices, with particular reference to the elderly.
* Ability to do physical assessments and develop plans of care.
* Knowledge of health counseling techniques and practices.
* Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
* Excellent organization and documentation skills.
* Familiarity with community health resources, which are available to participants and families.
* Skilled in developing and maintaining department quality assurance.
* Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
* Ability to react calmly and effectively in emergency situations.

Important Notes
Must be a Graduate of a school of professional nursing, BSRN preferred.
Current California Registered Nurse (RN) license, MSW or OT degree accepted.
CPR certification preferred.

A minimum of one year‚s experience in working with frail or elderly population required. Two years of management experience in a hospital, nursing home or community-based setting preferred. Shall have either training or related experience in the job assigned.
